A save where the game will announce the fortress has reached mountainhome status and that the monarch has arrived, only to have the monarch's entourage spawn alone. It's saved a fair number of days before the announcement, but obviously the requirements are already met. Whatever triggers the missing monarch bug definitely occurs before the monarch actually arrives.
