An influx of ~30 migrants with pages upon pages of absent relatives went unhappy->stressed->haggard-> constantly flipping out over the course of one year. One tantrum resulted in dwarven death and the fighting hasn't stopped since.

Additional note: the justice screen is only aware of disorderly conduct and vandalism, blithely ignoring the ~7 murders.
